1 1/2 oz VuQo Vodka
3/4 oz banana liqueur
3/4 oz Creme De Cacao
1/2 ripe banana
4 pcs. ChocNut or other milk chocolate
2 scoops vanilla ice cream
1/2 cup crushed ice
